Hello. May I please request a copy of my ITR for FY 2011? I have lost my copy and my previous employer couldn't provide me with a copy as well since as they have archived the records already. 

At the very least, can I get a statement from your office indicating that I paid tax for the year 2011 indicating the name of my employer?

I appreciate your assistance on this matter.

I refer to my letter of Jun 13, 2021 08:09:18 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR 2316.

In order for us to work further on your request, we need more information from you regarding your query.  In particular, it would be helpful to know your employer's TIN, business name & address from 2011 and email it at ana.marie.du@bir.gov.ph for us to properly endorse your request to the concerned RDO. 

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sept 06, 2021 03:35:24 AM, your request will be closed.
