Dear Ms. Reyes:

I refer to my letter of Jun 19, 2023 03:43:20 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR form No. 2316.

Per our phone conversation today, June 19, you were informed to provide the following details:

1. Complete Name of your Employer (For Calendar Year 2022);
2. TIN of Employer;
3. Registered Address of your Employer;
4. Your Date of Birth;
5. Your Address;
6. Your TIN; and
7. Your contact number (mobile no.) in case we need to call you for further info. 

Kindly email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the subject email ticket number #BIR-656671808507, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sep 11, 2023 08:07:57 AM, your request will be closed.
