Dear Ms. Del Rosario:

I refer to my letter of Jan 29, 2023 08:55:49 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for copy of my Income Tax Return (ITR) and/or BIR Form 2316.

In order for us to work further on your request, we need more information from you regarding your query and it would be helpful to know the following details:

1)	What kind of Income Tax Return did you file, is it BIR Form 1701? or 1701A?
2)     When did you file your Income Tax Return?
3)     What is the Complete name indicated in the Income Tax Return you filed?
4)	What is your Tax Identification Number as indicated in your filed return?
5)	What Revenue District Office No. did you file your form 1701 or 1701A?
6)	Did you file online or over the counter?

Please be informed further that the Revenue District Office (RDO) does not have  a copy/ies of the “No Payment” returns filed either through eBIRForms or eFPS.

Kindly email the needed information at sherry.ann.arcita@bir.gov.ph and indicate in the subject email ticket number #BIR-659882673762, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Apr 24, 2023 10:21:32 AM, your request will be closed.
