October 11, 2022 Madam: Greetings! Thank you for your request dated Oct 10, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Tin Verification 1904. Response to your request We called your mobile number today to validate your existing FOI request but it was unsuccessful. Upon verification of your TIN record it was registered under Revenue District Office (RDO) No. 113-West Davao City. If you need your BIR Form 1904 with stamped, the aforesaid RDO will be the one to attend to your request. We would like to advise you to secure a TIN VERIFICATION SLIP, kindly visit the nearest BIR Revenue District Office (RDO) on your request. Please bring two (2) valid government-issued I.D.s containing the photograph and signature as proof of your identity for validation purpose. Addresses and contact numbers of RDOs can be found in the BIR Website at www.bir.gov.ph homepage/contact us/directory/revenue district offices links. If the request is made through representative, a Special Power of Attorney of the requesting party (principal) and at least two (2) valid government IDs containing the photograph and signature of the representative and the principal shall be presented. Please be reminded that TIN is considered a piece of sensitive personal information that is bounded by the Data Privacy Act of 2012. Thank you.
