Dear Sir/Madam,

Greetings! Hope you are doing well.

I am writing in through this website to request and hopefully receive a copy of my BIR 2316 and 1905 forms for the years 2019 to 2020. This is for the purpose of submitting some employment requirements from my new employer. Regrettably, my previous employer is not able to provide me the documents mentioned as they have already released it previously and I have since overlooked/misplaced the documents.

I refer to my letter of Nov 22, 2022 12:27:27 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request for a Copy of my BIR Form 2316 and BIR Form 1905.

Kindly provide us the following data in order for us to evaluate your requested information:
1. Your Tax Identification Number (TIN); 
2. Name of your Employer/s (for Calendar Year 2019-2020);
3. TIN of your Employer/s; and
4. Address of your employer/s.

BIR Form 1905, on the other hand, is accessible at the BIR Website (www.bir.gov.ph).

Kindly email the requested information at sherry.ann.arcita@bir.gov.ph as this contains sensitive information and indicate in the email subject tracking number #BIR-674062604707, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Feb 14, 2023 05:17:00 AM, your request will be closed.
