This is [Redacted][Name], I would like to request a certified copy of BIR 2316 from my previous employer [Redacted][Organization] Because i lost my copy and my employer didn't provide my request. 

I appreciate your assistant on this matter.

Dear Ms. Potutan,

I refer to my letter of Mar 28, 2022 10:11:13 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Certified True Copy of BIR Form 2316.

In order for us to properly endorse and work further on your request, kindly provide us the TIN, RDO and complete name and address of your employer from 2016-2018.

On the other hand, it is also advised that you better ask for a copy of your 2316 directly from your employer as they are the main source of such documents.  It is also mandated by law that every employer shall issue BIR Form No. 2316 to every employee before the close of such calendar year, or if employment is terminated, on the day on which the last payment of compensation is made.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jun 20, 2022 06:59:47 AM, your request will be closed.

We hope to hear from you at the soonest possible time.
