I refer to my letter of Jan 10, 2024 12:46:28 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR FORM 1701.

In reply, kindly provide the following details:

1. Your complete name (including your middle name);
2. Your Date of Birth; and
3. Your Taxpayer Identification Number (TIN).
