I would like to request for my Form 2316 as it is part of my current pre-employment requirement to my soon new employer.

Dear Kevin Carl Perez:

I refer to my letter of Aug 01, 2022 06:54:30 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR Form 2316 for 2019 to 2022.

Kindly provide us the following data in order for us to evaluate your requested information and email the same at sherry.ann.arcita@bir.gov.ph, as this contains sensitive information. Indicate the  tracking number #BIR-686554993284, in the email subject. 

1. Your Tax Identification Number (TIN);
2. Name of your employer/s;
3. TIN of your employer/s;
4. Address of your employer/s;

Moreover, your employer’s submission of their employees’ BIR Form 2316 which should have been done on the Revenue District Office (RDO) where your employer is registered. On the other hand, you may also ask your employer/s for the given period for the copy/ies of your Certificate of Compensation Payment/ Tax Withheld (BIR Form 2316) because all taxpayers are required to preserve their accounting records for the period of 10 years in accordance with the Revenue Memorandum Circular No. 29-2019.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Oct 24, 2022 12:38:47 AM, your request will be closed.
