I am writing this letter to request a Certificate of Non-filing of Income Tax Return of my mother, Maria Lilibeth R. Biago, who is currently unemployed in order to complete a prerequisite for my application for the SM College Scholarship.

Greetings!

Thank you for your request dated Jan 18, 2024 10:17:03 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Certificate of Non-filing Income Tax Return.

Response to Request

This is in connection with your request for information, with reference number #BIR-709215853120, for the Requirements for Scholarship Application.

After careful consideration, provided hereunder are the Checklist of Documentary Requirements (For those Persons with Low Income/No Income) who may want to avail of the Scholarship and Job/Livelihood Programs:

1.	Certification of Low Income/No Income signed by the Barangay Chairman of the place where the applicant resides;
2.	Duly Notarized Affidavit of Low Income/No Income from the Notary Public;
3.	Request Letter;
4.	Photocopy of Identification Card with three (3) specimen signature;
5.	Payment of Certification Fee (P100.00) and Loose Documentary Stamp Tax (P30.00), per copy;
6.	If transacting through a representative, a Special Power of Attorney (SPA) shall be executed by the taxpayer-applicant and Any government-issued IDs of the Taxpayer and Authorized Representative.

Submit the above mentioned documents/requirements at the Collection Section of the
Revenue District Office (RDO) having jurisdiction over the place of Residence of the Taxpayer.
