I refer to my letter of Sep 08, 2022 02:30:58 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR Form 2316 and TIN Verification Slip.

Kindly provide us the following data in order for us to evaluate your requested information:
1. Your Tax Identification Number (TIN)
2. Name of your Employer/s (for 2020 and 2021)
3. TIN of your Employer/s
4. Address of your Employer/s

Email the needed information at sherry.ann.arcita@bir.gov.ph as this contains sensitive information and indicate in the subject email tracking number #BIR-719150017472, to respond.

On your other concern on TIN Verification Slip, we would like to advise you to visit the nearest Revenue District Office (RDO) on your request. Please bring two (2) valid government-issued I.D.s containing the photograph and signature as proof of your identity for validation purpose. Addresses and contact numbers of RDOs can be found in the BIR Website at www.bir.gov.ph homepage/contact us/directory/revenue district offices links. 

Please be reminded that TIN is considered a piece of sensitive personal information that is bounded by the Data Privacy Act of 2012.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Dec 02, 2022 06:04:01 AM, your request will be closed.
