I was wondering if you could help me obtain the fully accomplished 2316 BIR forms for the years 2017, 2018, and 2019, as I seem to have misplaced them. Kindly inform me of the appropriate email address to which I should forward my information. Your prompt response would be highly appreciated. Thank you!

I refer to my letter of Dec 07, 2023 07:26:35 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR Forms 2316 2017-2019.

In reply, we would like to inquire some details, to wit:

1.  What Income Tax Return are you referring to?
2. Did you have a registered business the mentioned Calendar Year (s) 2017 to 2019?
3. Or are you an Employee (Earning Purely Compensation Income) from the aforementioned Calendar Year (s)?
4. If employed, What is the Name of your Employers during the Calendar Year (s)?
5. Employers Taxpayer Identification Number (TIN)?
