Dear Sir/Madam,

Hope you are doing well.

I am writing in through this website to request and hopefully receive a copy of my BIR 2316 form. This is for the purpose of submitting one of the employment requirement for my new employer.  I worked from April 2018 to October 2021.

It would be very much appreciated if you can send me a copy via email as my employment is on the line — I have to submit this on July 1st.

Dear Ms. Justalero,

I refer to my letter of Jan 28, 2023 02:34:19 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request a copy of my BIR 2316.

Kindly provide us the following data in order for us to evaluate your requested information:

1. Your Tax Identification Number (TIN);
2. Complete Name of your Employer (For Taxable Year 2019-2021);
3. TIN of your Employer; and
4. Address of your Employer.

Email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the email subject ticket number #BIR-724699158030, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Apr 24, 2023 02:32:08 AM, your request will be closed.
