Hi po!

May I request for a copy of my BIR Form 2316 for the years 2020 and 2021.   My TIN is [Redacted][Name]. I worked for Teletech Iloilo at the time.  I am no longer associated with the company and tried to request from them or even asked for their TIN but am not getting any response as of the time of writing this request.

I will be using it as part of my kids' requirements in school.

Maraming salamat po and I appreciate that we have this option to request for our records.

I refer to my letter of Sep 09, 2022 03:08:45 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR FORM 2316.

Kindly provide us the following data in order for us to evaluate your requested information:
1. Your Tax Identification Number (TIN);
2. Complete Name of your Employer (for 2020-2021);
3. TIN of your Employer; and 
4. Address of your Employer.
