In order for us to work further on your request, we need more information from you regarding your query. In this case, we will be able to verify the requested document. In particular, it would be helpful to know your employer's TIN, RDO for the year 2009, 2010-2017, business name and address. In addition, we also require certification from your previous employer stating that your included in the transmittal received by the District Office. Moreover, for the year 2015-2017, "Annex F" from your previous employer stamped by the District Office is needed as supporting document for us to properly verify your request. Thank you for your reply.
I have sent the informations on employer's TIN, RDO for the year 2009, 2010-2017, business name and address at ana.marie.du@bir.gov.ph. Do you have an email so i can send this information or would replying here be okay as these contain sensitive information?
As for the certification, this would just be a certification letter from previous employer stating that I’m included in the transmittal received by the District Office right?
Lastly, would Annex F still be required if i’ll just request for the year 2009 to 2014?
