Madam:

Thank you for your request dated Apr 17, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, request for a copies of your BIR Form 2316 from Calendar Years 2015 to 2019.

In reply, please be informed of the requested BIR Form 2316 for the following Calendar Years (CY) dates:

1. CY 2015 to 2016 - This was endorsed and forwarded to the Office of Regular LT Audit Division 1 for consideration and appropriate action. Since Watsons Philippines Inc. is under their jurisdiction. They will provide the needed documents for the aforesaid Years if Watson Philippines Inc. has filed in your behalf;

2. BIR Form 2316 for CY 2017 - Please take note that for the year 2017 you are supposed to file your Annual Income Tax Return (ITR) since you have two (2) Employers namely Watsons Philippines Inc. and Ma. Lingad Memorial Hospital, you are likewise have to file your Annual ITR using BIR Form 1700 through eBIR Forms (Apps downloadable at the BIR website, www.bir.gov.ph), attached and consolidate your 2316 from your employers.

3. BIR Form 2316 for CY 2018 - the record of the Revenue District Office (RDO) No. 28-Novaliches (where Ma. Lingad Memorial Hospital is registered) on those periods are already unavailable since we follow strictly the four (4) year retention period based on existing revenue issuances of the BIR (Revenue Memorandum Circular 73-2008).

4. BIR Form 2316 for CY 2019 -this will be endorsed to RDO No. 28-Novaliches for consideration and appropriate action.

Lingad Memorial General Hospital for TY 2017-2019. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
