Good day,

I hope this letter finds you well. I am writing to request a copy of my Income Tax Return (ITR) for the financial year 2022 as I have lost the only copy I had last time. I am under Income From Profession - Graduated IT Rates.

My name is Florenz Keirt M. Cuizon, and my tax identification number is 499186290. I am in need of a copy of my ITR for my housing loan application.

I would be grateful if you could provide me with a copy of my ITR at the earliest convenience.

Dear Mr. Cuizon:

I refer to my letter of Jan 09, 2024 11:04:58 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request for a Copy of Income Tax Return (ITR) and/or BIR Form No. 2316.

In reply, you are to coordinate with your EMPLOYER as the same is Mandated to Issue the BIR Form No. 2316 for the Calendar Year 2022, pursuant to Section 83 of the 1997 Tax Code and Section 2 [2.83.1] of Revenue Regulations (RR) No. 3-2002, respectively. 

The EMPLOYER shall furnish each Employee/s with the Original Copies of BIR Form No. 2316 as Proof that your Taxes were Withheld and this will serve as your Income Tax Return (ITR).  EMPLOYER is required to issue BIR Form No. 2316 on or before January 31 of Every Year and On the Day the Last Payment of Compensation is Paid.
