I would like to request a copy of my previous latest Income Tax Return (2021-2022). I could not return to my previous employer as I already relocated, so I couldn't get a copy of my latest ITR from them.
Here are my details for your reference:
Previous Company: WESTERN INSTITUTE OF TECHNOLOGY
Address: LUNA ST., LA PAZ, ILOILO CITY
Position: FACULTY
Years worked: JUNE 2018 - JUNE 2022
*My only source of income during these years was from my teaching job at this school.

In order to identify or locate the requested information, we are seeking for further clarification specifically:
1. Were you an Individual Earning on Purely Compensation Income from the
period June 1, 2021 to June 1, 2022 from your last employer? or
2. Were you registered as a Job Order Contract or Contract of Service from the
period June 1, 2021 to June 1, 2022 from your last employer?
3. Can you provide the Taxpayer Identification Number (TIN) of your employer,
so we can coordinate and endorse your request to the Revenue District Office
(RDO) where your employer is registered.
