Good day,

My name is Paul Anthony Uy, I have work in Milagros Albano District Hospital as a nurse under the Management of Provincial Government Of Isabela from January 2016 to June 2019. I would like to request from your good office copies of my Income tax Return on the said years. I need my ITR for proof of my service in the said hospital, attached is my TIN # 481-217-217. If you could send me a copy of this document on my email: paulanthonyuy@gmail.com.

Dear Mr. Uy:

I refer to my letter of Aug 12, 2023 12:14:43 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return ( ITR) and/or BIR Form No. 2316.

In order to identify or locate the requested information, we are seeking for further clarification specifically:

1. Did you have a Registered Business between the period 2016 to present? Or were you an Individual Earning on Purely Compensation Income from the period 2016 to present?

2. Withstanding your answer to the first and second query, did you file an ITR for the cited period?

As an added information, if you are an Employee for the period 2016 to present, your EMPLOYER is required to issue a copy of your BIR Form No. 2316 for Calendar Year 2016 to 2023 (in case your employment with your employer has been terminated before the end of this year) pursuant to Section 2.83.1 of Revenue Regulations No. 02-98, as amended which provides that “The EMPLOYER shall furnish each employee with the original copies of Form No. 2316 xxx.” As proof that your taxes were withheld. This will serve as your Income Tax Return (ITR).

Please be informed also that the available copy of documents in the Revenue District Office (RDO) are from Calendar Year 2016 to 2023.
