Response to Request
We called your mobile number today, December 28 and apprised you that your concern/request can be obtained from Revenue District Office (RDO) 55 - San Pablo City, East Laguna located at BIR Regional Office Building Barangay San Nicolas Maharlika Hi-way, San Pablo City. We advised you to fill our the BIR Form 1904 with attachment of your Birth Certificate and Barangay Clearance for bank purposes.
