In order for us to work further on your request, it would be helpful to know the following details:

1. what kind of Income Tax Return Form you are requesting from us? Is it BIR Form No. 1701 or BIR Form No. 1701A?

2. What is the Complete Name indicated in the Income Tax Return you filed?
3. What is your Tax Identification Number (TIN) as indicated in your filed 
     Return?
4. What Revenue District Office No. did you file your Income Tax Return Form?
5. Did you file Online (thru eBIR Forms facility or Manually at the Revenue District Office?
