Dear Ms. Cruz:

I refer to my letter of Feb 01, 2023 02:20:14 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request BIR Form No. 1701.

In order for us to work further on your request, please be informed that BIR Form 1701 is the Income Tax Return (ITR) filed by individuals who are engaged in trade/business or the practice of profession including those with mixed income (i.e., those engaged in the trade/business or profession who are also earning compensation income) in accordance with Sec. 51 of the Code, as amended, and will serves as your Income Tax Return). 

In particular, it would be helpful to know the following details:
1)	What is the Complete name indicated in the Income Tax Return you filed?
2)	What is your Tax Identification Number as indicated in your filed return?
3)	What Revenue District Office No. did you file your form 1701?
4)	Did you file it online or over the counter?

Please be informed further that the Revenue District Office (RDO) does not have copies of the “No Payment” returns filed either through eBIRForms or eFPS.

Kindly email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the subject email ticket number #BIR-788341987159, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Apr 26, 2023 07:38:07 AM, your request will be closed.
