Dear Ms. Alfonso:

I refer to my letter of Feb 14, 2023 06:32:11 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return (ITR).

In order for us to work further on your request, we need more information from you regarding your query.  In particular, it would be helpful to know the following details:
1)	What is the Complete name indicated in the Income Tax Return you filed?
2)	What is your Tax Identification Number as indicated in your filed return?
3)     What Income Tax Form did you file, is it BIR Form 1701 or 1701A?
4)     Exact Date (Month, Day and Year) of filing of your Income Tax Form?
4)	Did you file online or over the counter?

Please be informed that the Revenue District Office (RDO) does not have the copies of the “No Payment” returns filed either through eBIRForms or eFPS.

Email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the subject email  ticket number #BIR-797294872631, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before May 09, 2023 11:42:28 PM, your request will be closed.

The name indicated in my ITR is ROSE MAURELLE C. ALFONSO, with tax identification number 735-794-700-000. I filed BIR Form 1701 last April 14, 2021 via eBIRForms offline.
