Thank you for your request dated Jul 30, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You request for a TIN VERIFICATION SLIP.

In reply, We would like to advise you to visit the nearest Revenue District Office (RDO) on your request. Based on your given address, you may proceed with RDO No. 26-Malabon-Navotas located at 5th Floor Fisher Mall 64 Road Brgy. Longos Malabon City. Please bring two (2) valid government-issued I.Ds containing the photograph and signature as proof of your identity. If the request is made through representative, a written authorization and/or special power of attorney of the requesting party (principal) and at least two (2) valid government IDs containing the photograph and signature of the representative shall be presented.

Please be reminded that TIN is confidential information that is bounded by the Data Privacy Act of 2012.
