Greetings!

Thank you for your request dated Apr 28, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You request for a copy of your Income Tax Return for Visa Application Purpose.

In response to your request, you should file BIR Form 1700 Annual Income Tax Return with attached proof of income, BIR Form 2316 Certificate of Income and Tax Withheld issued by your employer to the Revenue District Office (RDO) where you are registered.

Moreover, if you have filed already filed BIR Form 1700 but have lost your copy, you may request a copy of the same from the concerned RDO.
