Due to Data Privacy Act of 2012. Request for Certified True Copy of ITR should be done personally or thru a duly authorized representative at the concerned Revenue District Office. The following are the requirements to be submitted to the Collection Section. For more inquiries, you may contact them thru 0968-703-9930.

1.	Request letter stating the purpose
2.	Photocopy of ID of requestor
3.	SPA (if transaction will be done by a representative)
4.	Filled-out request for certified true copy from Counter 9
5.	ID of Authorized representative
