Greetings!

Thank you for your request dated Dec 08, 2022 08:24:10 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for a Certified True Copy of BIR Form 2316.

Response to Request

If you need a Certified True Copy (CTC) of your BIR Form 2316 for the year 2021, you may personally or through your authorized representative file your request to the Revenue District Office (RDO) having jurisdiction over the place of employer.

You can also ask from your employer copy of the Duly Received “Annex F” (Proof that your employer came to Revenue District Office) and have the List & BIR Form 2316 of their employees who availed of substituted filing. Then bring a copy of 2316 of the requesting party for stamping of certified copy.

Any request for certification that may be requested by a taxpayer from the RDO on matters relating to its registration shall be charged with a fee in an amount not exceeding one hundred pesos (P100.00), in addition to the documentary stamp tax imposed under Section 188 of the National Internal Revenue Code, as amended.
