I would like to request a copy of the prescribed/required format for the Schedule of Depreciation to be submitted with the BIR. 

I would appreciate it if you will also provide the revenue issuance recommending the prescribed format.

You asked for Prescribed format for Schedule of Depreciation.

Response to Request

Per our telephone conversation today, November 8, you have been apprised - for reference you can check Revenue Regulations No. 12-2012 which is available in the BIR Website (www.bir.gov.ph) under Revenue Issuances.

As to the prescribed format for the Schedule of Depreciation to accompany the Financial Statements and Income Tax Return to be submitted to BIR. We hereby  advise to coordinate with the Officer-of-the-Day of the Revenue District Office (RDO) where registered for assistance.
