In order for us to work further on your request, we need more information from you regarding your query. In particular, it would be helpful to know the following details:

1)	What kind of Income Tax Return (ITR) Form are you requesting from us? Is it 
        BIR Form 1701 or BIR Form 1701A? Kindly check your Certificate of 
        Registration since you are registered as Single Proprietor.
2)     What is the Complete name indicated in the Income Tax Return you filed?
3)	What is your Tax Identification Number as indicated in your filed return?
4)	What Revenue District Office No. did you file your form 1701 or 1701A?
5)	Did you file it Online (via eBIR Form facility) or over the counter?
