This form is needed for me to apply in TOPLC Scholarship partnered by my school, Manila Central University. Thank you!

Greetings!

Thank you for your request dated Jan 29, 2024 03:01:14 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Certificate of Low Income for my mother.

Response to Request

This is in connection with your request for information, with reference number #BIR-857507349965, for the Requirements for Scholarship Application.

After careful consideration, provided hereunder are the Checklist of Documentary Requirements (For those Persons with Low Income/No Income) who may want to avail of the Scholarship and Job/Livelihood Programs:

1.	Certification of Low Income/No Income signed by the Barangay Chairman of the place where the applicant resides;
2.	Duly Notarized Affidavit of Low Income/No Income from the Notary Public;
3.	Request Letter;
4.	Photocopy of Identification Card with three (3) specimen signature;
5.	Payment of Certification Fee (P100.00) and Loose Documentary Stamp Tax (P30.00), per copy;
6.	If transacting through a representative, a Special Power of Attorney (SPA) shall be executed by the taxpayer-applicant and Any government-issued IDs of the Taxpayer and Authorized Representative.

Submit the above mentioned documents/requirements at the Collection Section of the
Revenue District Office (RDO) having jurisdiction over the place of Residence of the Taxpayer.
