I am Niña Mae Birao and I would like to request for Certificate of Non-filing of Income tax return of my mother, Nilda E. Birao, who is currently unemployed. I need this document with my application for SM Scholarship Program 2024.

Greetings!

Thank you for your request dated Feb 02, 2024 04:52:48 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Request for Non Filing of Income Tax Return.

Response to Request

After careful consideration, provided hereunder are the Checklist of Documentary Requirements (For those Persons with Low Income/No Income) who may want to avail of the Scholarship and Job/Livelihood Programs:

1.	Certification of Low Income/No Income signed by the Barangay Chairman of the place where the applicant resides;
2.	Duly Notarized Affidavit of Low Income/No Income from the Notary Public;
3.	Request Letter;
4.	Photocopy of Identification Card with three (3) specimen signature;
5.	Payment of Certification Fee (P100.00) and Loose Documentary Stamp Tax (P30.00), per copy;
6.	If transacting through a representative, a Special Power of Attorney (SPA) shall be executed by the taxpayer-applicant and Any government-issued IDs of the Taxpayer and Authorized Representative.

Submit the above mentioned documents/requirements at the Collection Section of the
Revenue District Office (RDO) having jurisdiction over the place of Residence of the Taxpayer.

Thank you so much for the response. I would like to ask some questions. Is it okay if the documents that will be given and provided from Bacolod City Negros Occidental? My mother is living there and I'm currently living with my sister here at Caloocan.
