Good day Ma'am/Sir!

I hope this letter finds you well. My name is Rebecca C. Casio, and my tax identification number is 007291508. I am writing to request a copy of my Income Tax Return (ITR) for the financial year 2020-2023. During the mentioned years, I worked as a private school teacher in Lapu-Lapu City, Cebu.

 I am in need of a copy of my ITR for my visa application.

I would be grateful if you could provide me with a copy of my ITR at the earliest convenience.

Dear Mr. Casio:

I refer to my letter of Jan 21, 2024 06:59:01 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request for a Copy of Income Tax Return (ITR).

In reply, BIR Form No. 2316 is a form issued by your employer (A Certificate). Hence, you may request for the issuance and/or re-issuance of the said documents to the issuing company. You must be issued of the copy of the said form on or before January 31 of the succeeding year.

With the employer's obligation to give you the original copy of the form, it is therefore your right to request said copy from your employer. 

While our aim is to provide information whenever possible, in this instance the Revenue District Office (RDO) where your employer is registered would take longer to provide you the information you have requested. They have to verify if your employer has submitted to them the List of Employees Qualified under substituted Filing of Income Tax Return (Annex F). Furthermore, the date of filing should also be given for the RDO to be able to verify at the fastest possible time.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Apr 17, 2024 02:16:35 AM, your request will be closed.
