I am writing in through this website to request and hopefully receive a copy of my BIR 2316 form. This is for the purpose of submitting one of the employment requirement for my new employer. 

It would be very much appreciated if you can send me a copy via email as my employment is on the line. I have to submit this on May 17, 2023.

I refer to my letter of May 13, 2023 04:49:52 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for 2316 BIR form.

In reply, kindly provide the following;

1. Name of your Employer for Calendar Year 2017 to 2022;
2. TIN of Employer;
3. Registered Address of your Employer;
4. Your Birthday;
5. Your Address; and
6. Your TIN.

Email the needed information at vanessa.badeo@bir.gov.ph and indicate in the email subject ticket number #BIR-863823089608, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Aug 07, 2023 06:50:00 AM, your request will be closed.
