I refer to my letter of May 20, 2023 06:36:43 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for ITR 2316.

In order for us to work further on your request, we need more information from you regarding your query. Relative to this, kindly provide the following;

1. Name of Employer/s for Calendar Year/s 2022 to 2023;
2. TIN of Employer/s;
3. Registered Address of your Employer/s;
4. Your Birthday;
5. Your Address;
6. Your TIN; and
7. Your contact number (mobile no.) in case we need to call you for further information.
