Dear Sir/ Madam, 

I would like to request your good office for a Certified true copy of my ITR (BIR form 2316) 1filed for the calendar years 2020 and 2021. 

The said copies will be used for my employment as my new employer is needing such for background checking purposes.

Dear Ms. Guela:

I refer to my letter of Oct 07, 2023 12:22:48 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for a Certified True Copy of your BIR Form 2316.

If you need a Certified True Copy (CTC) of your BIR Form No. 2316 for the Calendar Year (s) 2020 and 2021, you may personally or through your Authorized Representative file your request to the Revenue District Office (RDO) having jurisdiction over the place of your Employer (s).

Kindly bring the Original and Photocopy of your BIR Form No. 2316 and you may also ask from your Employer (s) copy of the Duly Received “Annex F”. 

Any request for certification that may be requested by a taxpayer from the RDO on matters relating to its registration shall be charged with a fee in an amount not exceeding one hundred pesos (P100.00), in addition to the documentary stamp tax of Thirty Pesos (P30.00) imposed under Section 188 of the National Internal Revenue Code, as amended.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jan 01, 2024 06:10:53 AM, your request will be closed.
