My previous employers for the year 2019 are as follows:
1. Calamba Doctors' Hospital.
2. Mercury Drug Corporation.

For reference, I am requesting for a copy my BIR 2316 for the year 2019 because I want to close my registration in BIR RDO 53B as a Job Order. I have open cases and one of the documents that I need to submit is my BIR 2316 for the year 2019.

I received a call regarding my request. However, the call was cut because of the poor signal. If I need to file 1700 because I have two previous employers for the same year, I would still like to pursue my request for a copy of my BIR 2316 (Year 2019). This is to properly file my 1700 and to also properly close my open cases in RDO 53B.
