Thank you for your request dated Feb 16, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Form 2316.

Please be informed that your request was endorsed to the Revenue District Office (RDO) No. 98-Cagayan De Oro City for consideration and appropriate action. 

This is also the RDO where your employer (for 2019) was registered. 

Please be informed also, that the RDO (Compliance Section) will be requiring you to submit Annex F (Certified List of Employees Qualified for Substituted Filing of ITR/BIR Form 2316 per Revenue Regulations No. 11-2018.

This has reference to you request for a copy of your BIR Form No. 2316 for Taxable Year 2019. Before granting your request, may we asked for a copy of the submitted Annex F - Certified List of Employees Qualified for Substituted Filing of Income Tax Return thru BIR Form 2316, which you may secure for your employer so we can validate if the same requested document/form (i.e., BIR Form 2316) has been duly endorsed in our office. Nonetheless, you may visit our office and proceed with our Compliance Section so we can directly accomodate your concern. Thank you.
