I am Sophia Antonette V. Forslund, and i am requesting a Certificate of Non Filling of Income of my mother, Arlene Venturina Alquiroz, who is currently unemployed. I need the document for my SM Scholarship application.

Greetings!

Thank you for your request dated Feb 01, 2024 08:50:35 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Certificate of Non Filling of Income.

Response to Request

This is in connection with your request for information for the Requirements for Scholarship Application.

After careful consideration, provided hereunder are the Checklist of Documentary Requirements (For those Persons with Low Income/No Income) who may want to avail of the Scholarship and Job/Livelihood Programs:

1.	Certification of Low Income/No Income signed by the Barangay Chairman of the place where the applicant resides;
2.	Duly Notarized Affidavit of Low Income/No Income from the Notary Public;
3.	Request Letter;
4.	Photocopy of Identification Card with three (3) specimen signature;
5.	Payment of Certification Fee (P100.00) and Loose Documentary Stamp Tax (P30.00), per copy;
6.	If transacting through a representative, a Special Power of Attorney (SPA) shall be executed by the taxpayer-applicant and Any government-issued IDs of the Taxpayer and Authorized Representative.

Submit the above mentioned documents/requirements at the Collection Section of the
Revenue District Office (RDO) having jurisdiction over the place of Residence of the Taxpayer.
