Dear Mr. Palma Jr.:

I refer to my letter of Jul 04, 2023 11:31:54 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return (ITR).

In order for us to work further on your request, it would be helpful to know the following details:

1)	What kind of Income Tax Form are you referring to, is it BIR Form No. 1701? 
        or 1701A (kindly check your Certificate of Registration)?
2)     What is the Complete Name indicated in the Income Tax Return you filed?
3)	What is your Tax Identification Number (TIN) as indicated in your filed 
        return?
3)	What Revenue District Office No. did you file your form 1701 or 1701A?
4)	Did you file online (thru eBIRForms facility) or over the counter?

Please be informed further that this office does not have copies of the “No Payment” returns filed either through eBIRForms or eFPS.

Kindly email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the subject email ticket number #BIR-908391975568, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sep 26, 2023 06:29:53 AM, your request will be closed.
