To Whom It May Concern,

I am writing to request a copy of my BIR 2316 from my previous employment at ACI, Inc, formerly known as Araneta Center Inc, for the period from April 2015 to July 2019. I resigned in April 2019, and I have not yet received my BIR 2316. I require this document as proof of employment for inclusion in my application to work abroad.

It would be appreciated if I could receive copies for both 2015 and 2019, as they are part of my current requirements.

Thank you for your prompt attention to this matter.

In reply, your request was already been endorsed to the concerned office the Regular LT Audit Division III (the office that has jurisdiction over your previous employer). They will be providing you the said BIR Form No. 2316 if your previous employer has filed in your behalf and your name was included in the Annex F (Certified List of Employees Qualified Under Substituted Filing).
