I refer to my letter of Feb 23, 2023 01:40:46 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, request for a copy of your BIR Form 2316 for the Year 2020.

In order for us to work further on your request, we need more information from you regarding your query. Kindly provide us the following information:

1. Complete Name of your Employer for Taxable Year 2020;
2. TIN of your Employer for us to properly endorse your request to the Revenue District Office (RDO) where your employer is registered; and
3. Complete Address of your Employer;
4. The RDO will also ask for a copy of the Certified List of Employees Qualified for Substituted Filing of Income Tax Return (ITR) - Annex F of Revenue Regulations No. 11-2018.
