I am Simon Aqui N. Diaz, a former government employee. I would like to request for a copy of my BIR Form 2316 for the year 2022. I have been furnished by my employer with a copy of the said form, but I lost the copy in my possession despite efforts to retrieve it. Hence, I am submitting this request. The following are relevant information you may need for this request:

2. Name of Employer in 2022: Public Attorney's Office (PAO)
3. TIN of Employer: 001 270 688
4. Address of Employer: DOJ Agencies Bldg., NIA  Road, Diliman, Quezon City 1100

Dear Simon Aqui Diaz,

I refer to my letter of Dec 04, 2023 08:19:04 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for 2022 ITR 2316.

After careful consideration, copies of Income Tax Return (ITR) can be requested from the Revenue District Office (RDO) where the same was filed and/or submitted or to the RDO where your Taxpayer Identification Number (TIN) is registered. You just have to submit a Signed Letter Request for the purpose. However, if you are referring to BIR Form No. 2316, the same shall have to be requested from your EMPLOYER since the same is the One Mandated by Law to Prepare and Provide to the Concerned Employee/s pursuant to Section 2.83.1 of Revenue Regulations (RR) No. 10-2008.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Feb 27, 2024 01:39:36 AM, your request will be closed.
