Good day! 

My name is Antonio A. Bautista and I am the proprietor of a business in Baguio City. I would like to request for a copy of my Income Tax Return for the previous year for my family's visa application. The original copy we were provided with was misplaced during our renovation and we do not have any duplicates.

Should you need more information, please let me know. Appreciate your kind and prompt assistance.

I refer to my letter of Nov 21, 2023 06:52:21 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request for BIR 1701.
