Hi, 

I hope you are doing well. I would like to request for a copy of my 2316 for the year 2018 to 2019 and 2019 to 2020. 

I'll be using it for the SSS loan reconstruction application. I have tried requesting it from my previous employer but they haven't got back to me. 

Hope you can help with this matter.
