Dear Ms. Ladeza:

I refer to my letter of Dec 22, 2022 09:13:38 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Copy of Income Tax Return (ITR).

We called your mobile number today, December 23, to validate your existing eFOI request, but it was unsuccessful.

Please take note that you are registered as Professional Under Revenue District Office (RDO) No. 41-Mandaluyong City and you are supposed to file your own Annual Income Tax Return (AITR) using BIR Form 1701 or BIR Form 1701A through eBIRForms (Apps downloadable at the BIR website, www.bir.gov.ph).

Furthermore, the filing of the Annual Income Tax Return (AITR) is your personal obligation and not by your employer. 

For assistance, you may visit RDO 41-Mandaluyong City located at YSK Bldg, 743 Boni Avenue, Malamig, Mandaluyong City. Or call them at (02) 8531-0663 and (02) 8532-5317.  

You may use the reply function on the eFOI portal at https://www.foi.gov.ph/requests/aglzfmVmb2ktcGhyHQsSB0NvbnRlbnQiEEJJUi05OTIzNjExMjk1ODQM, for request with ticket number #BIR-992361129584, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Mar 17, 2023 02:10:27 AM, your request will be closed.
