I refer to my letter of Aug 07, 2022 12:16:21 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR FORM 2316.

In order for us to work further on your request, kindly provide us the Name, Business Address and TIN of your Employer for the period covered (2021) for us to coordinate and properly endorse your request to the Revenue District Office (RDO) where your employer is registered. 

Email the needed information at sherry.ann.arcita@bir.gov.ph as this contains sensitive information. Indicate in the subject email tracking number #BIR-995777707578, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Oct 28, 2022 06:02:04 AM, your request will be closed.

Dear Abbygail Busa,

Greetings!

Please be informed that per verification of our records, your name is not listed or included in the sworn declaration filed by Trends Group, Inc, for Taxable Year 2021 for employees qualified for substituted filing of Income Tax Return per Revenue Regulation No. 2-98, as amended.

In this regard, we cannot issue your BIR Form 2316 for Taxable Year 2021.
