I refer to my letter of Aug 02, 2023 10:44:06 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return and/or BIR Form No. 2316.

For consideration purpose, kindly provide the following:

1. Name of Employer for CY 2022;
2. TIN of Employer;
3. Registered Address of your Employer;
4. Your Date of Birth;
5. Your Address;
6. Your TIN; and
7. Your contact number (mobile no.) in case we need to call you for further info.
