Hi! I would like to request a copy of Bir Form 2316 from my last employer. It was one of the requirements of my new employer. It will be much appreciated if you can send it to my email. Thank you in advance.

In order for us to properly endorse and work  further on your request, kindly provide your employer’s TIN, RDO no., business name & address (from 2003-2009)and email them at ana.marie.du@bir.gov.ph. as these contain sensitive information
