In reply, BIR Form 1904 is available for download at the BIR Website. You can print the said form and have it stamped by RDO No. 03-San Fernando, La Union where your TIN is registered. The BIR district office is located at 2F TAN Building, National Highway, Sevilla San Fernando City, La Union.
