Hello, I am Raphael Amponin and I would like to ask for a copy of my 2316 for the year 2021. I had asked my previous employer numerous times but no response from them until now.

I refer to my letter of Sep 28, 2022 09:04:01 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR Form 2316 for the year 2021.

Kindly provide us the following data in order for us to evaluate your requested information:
1. Your Tax Identification Number (TIN);
2. Name of your employer;
3. TIN of your employer; and
4. Address of your employer.
