Dear Ms. Colorado:

I refer to my letter of Jun 19, 2023 01:31:07 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for For BIR Form No. 2316.

Relative to this, kindly provide the following:

Name of Employer (For Calendar Year 2020 );
TIN of Employer;
Registered Address of your Employer;
Your Date of Birth;
Your Address;
Your TIN; and
Your contact number (mobile no.) in case we need to call you for further information.

Your request will be endorse to the Revenue District Office (RDO) where your employer during the period is registered. They will provide you the needed documents if your employer has submitted the same in your behalf.

Kindly email the requested information at sherry.ann.arcita@bir.gov.ph as this contains sensitive information, indicate in the email subject ticket number #BIR-039408196764, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sep 11, 2023 12:48:36 AM, your request will be closed.
